What Is Depression? About Your Score
DEPRESSION AND ANXIETY SCREENING QUESTIONNAIRE

    The following questions ask about how you have been feeling in the last four weeks.
    When you have completed the questionnaire, calculate your score then print this page so that you can discuss the questions and your responses with your doctor or other health care professional.

    For each question, mark the box that best describes the amount of time you felt that way.
    There are two introductory questions to get you started.
    Once you have answered all the questions, press calculate and see what your score means.

In the past 4 weeks, how many days were you TOTALLY UNABLE to work, study or manage your day to day activities because of these feelings?

    If you have reported any days on which you were totally unable to work, study or manage your day to day tasks because of the feelings you have described above, you may benefit from getting some professional medical advice.
    Even if you believe that your distress is mainly due to physical health problems, it is still worth checking.
